fa بررسی سرولوژیک و اتیولوژیک تشخیص لپتوسپیروز در گاوهای استان مرکزی Serological and Etiological Investigation for Diagnosing Bovine Leptospirosis in Markazi Province دامپزشکی دامپزشکی پژوهشي Research <div style="text-align: justify;">چکیده مبسوط مقدمه و هدف: بیماری لپتوسپیروز که توسط سروتیپ‌های باکتری لپتوسپیرا اینتروگانس ایجاد می‌شود، یکی از خطرناک‌ترین بیماری‌های زئونوز به‌شمار می‌رود که طیف &lrm;وسیعی از نشانه &lrm;های بالینی تحت حاد تا حاد را ایجاد می‌کند، می&lrm;تواند سبب ناباروری، سقط جنین و کاهش تولید شود و خسارات اقتصادی زیادی به صنعت دامپروری کشور وارد نماید. تشخیص بیماری لپتوسپیروز، به &lrm;دلیل مخاطرات بالقوه &lrm;ی آن برای جوامع بشری، از اهمیت ویژه&lrm; ای برخوردار است، زیرا این بیماری به راحتی توسط میزبانانی نظیر موش و سگ که در اغلب موارد به تعداد قابل توجهی در محیط&lrm; های نگهداری دام&lrm; ها پیدا می&lrm; شوند، انتقال می &lrm;یابد. اما از آن &lrm;جایی&lrm; که این بیماری قادر است علائمی بسیار گمراه &lrm;کننده مانند زردی مخاطات، تورم غدد لنفاوی، التهاب عنبیه، خون&lrm; شاش، ورم پستان، افت شدید تولید شیر و تغییر رنگ آن، سقط جنین، تولد گوساله &lrm;های ضعیف، کاهش وزن و اختلال در باروری را در دام &lrm;ها ایجاد کند، احتمال افزایش خطا در تشخیص بیماری را از سوی دامپزشکان مهیا می &lrm;کند و به این &lrm;ترتیب وضعیت سلامت جوامع انسانی را تحت &lrm;تأثیر قرار می &lrm;دهد. برای تشخیص آزمایشگاهی بیماری از روش&lrm; های مختلفی مانند مشاهده مستقیم میکروسکوپی، کشت،PCR ، آزمایش آگلوتیناسیون میکروسکوپی(MAT) ، روش الیزا (ELISA) و ایمونوفلورسانس (IFA) می &lrm;توان استفاده نمود. هدف از این تحقیق مطالعه بررسی سه روش سرولوژی، کشت و PCR در تشخیص لپتوسپیروز گاوها بود. مواد و روش &lrm;ها: در این مطالعه از 400 راس گاو که حداقل سابقه یکی از علائم لپتوسپیروز را داشتند، نمونه &lrm;های&lrm; خون و ادرار برداشت شدند. خون&lrm;گیری از ورید وداج تحت شرایط بهداشتی انجام گرفت. از نمونه سرم &lrm;خون برای آزمایش سرولوژی (MAT) استفاده شد. جداسازی سرم &lrm;ها از لوله &lrm;های حاوی لخته &lrm;ی خون با قراردادن آن‌‌ها در بن&lrm;ماری  °C37 به &lrm;مدت 20 دقیقه، و سانتریفیوژ با سرعت g ×3000 و به &lrm;مدت 10 دقیقه انجام شد. به&lrm; منظور حفظ سرم &lrm;های جدا شده تا زمان شروع آزمایش MAT، سرم &lrm;ها توسط سمپلر به میکروتیوب &lrm;های استریل 2 میلی &lrm;لیتری انتقال داده شدند تا در دمای °C20- نگهداری شوند. برای انجام روش تست استاندارد MAT، آنتی &lrm;ژن مورد استفاده در این روش (لپتوسپیرا اینتروگانس) به&lrm; مدت 7 الی 10روز در یک محیط کشت مایع کشت داده شد. میزان حجم لپتوسپیرای مورد استفاده توسط شمارش اتاقک &lrm;ها مورد ارزیابی قرار گرفت و این حجم به &lrm;نحوی تنظیم شد که در هر میلی لیتر، 108×2 لپتوسپیرا وجود داشته باشد. از نمونه&lrm; های ادرار برای کشت ادرار و آزمایش PCR استفاده گردید. برای انجام آزمایش PCR، پرایمرهای باکتری لپتوسپیرا اینتروگانس مورد بررسی قرارگرفتند. گسترش واکنش &lrm;های PCR در حجم های واکنش 25 میکرولیتری با استفاده از مسترمیکس آماده (شرکت BIOFACT، کره، ddH2O: 5.50 µL, Buffer: 12.50 µL, Forward primer: 1.00 µL, Reverse  primer: 1.00 µL, DNA: 5.00 µL) انجام گرفت. حجم ادرار مورد نیاز 50 میلی &lrm;لیتر بود که این حجم در بطری &lrm;های حاوی 0/5 میلی &lrm;لیتر فرمالدئید فیلترشده (μm 0/45) جمع &lrm;آوری گردید. سپس به منظور انجام کشت و جداسازی باکتری لپتوسپیرا، 5 میکرولیتر از ادرار جمع &lrm;آوری در محیط&lrm; کشت اختصاصی باکتری (محیط کشت نیمه&lrm; جامد EMJH) تلقیح شد. محیط&lrm; های کشت حداقل به &lrm;مدت 5 هفته در درمای  °C30 انکوبه گردیدند و هر یک هفته در میان برای بررسی رشد لپتوسپیرا با استفاده از میکروسکوپ زمینه سیاه (DFM) مورد بررسی قرار گرفتند. داده‌ها با استفاده از آزمون کای‌اسکوئر و آزمون دقیق فیشر با استفاده از نرم‌افزار آماری (SPSS 23.0 (SPSS, Inc., Chicago, IL, USA 2007) تجزیه و تحلیل شدند. تفاوت‌های معنی‌دار بین میانگین‌ها در سطح معنی‌داری 0/05 > P در نظر گرفته شدند. یافته ها: تست MAT در پنج راس گاو (1/2 درصد) مثبت بود. سروتیپ&lrm; های ایکتروهموراژیه (2 راس= 40 درصد)، گریپوتیفوزا (2 راس=40 درصد) و کانیکولا (1 راس=20 درصد)  شایع &lrm;ترین بودند. هر پنج نمونه &lrm;ای که تست MAT آن&lrm;ها مثبت اعلام شد، به&lrm;&lrm; علاوه تمام گاوهایی که علائم بالینی لپتوسپیرووز را داشتند کاندید انجام آزمایش PCR شدند که در این میان، نتیجه چهار راس گاو (1 درصد) مثبت شد. برای تمامی گاوهای مشکوک که تست MAT، PCR و علائم بالینی مثبت داشتند، از نمونه &lrm;های ادرار، کشت تهیه شد. علی&lrm; رغم آلودگی دو نمونه از محیط کشت&lrm; ها به باکتری&lrm; های محیطی، هیچ یک از دیگر محیط &lrm;ها آلودگی نشان ندادند و رشد لپتوسپیرا در محیط ملاحظه نشد (صفر درصد). از آن&lrm; جایی&lrm; که حیواناتی که در مراحل اولیه بیماری هستند و سیستم ایمنی بدن آن&lrm;ها هنوز باکتری &lrm;ها را از خون به&lrm; طور کامل پاک نکرده است، باکتری&lrm; ها ممکن است زمان کافی برای کلونیزاسیون در کلیه پیدا نکرده&lrm; باشند و این منجر به نمونه ادرار منفی شده &lrm;است. همچنین، باکتری لپتوسپیرا به بسیاری از آنتی &lrm;بیوتیک &lrm;ها حساس است (درمان آن داکسی &lrm;سایکلین و آزیترومایسین است)، لذا در مواردی که دام بیماری دیگری دارد ممکن است این آنتی &lrm;بیوتیک &lrm;ها استفاده گردد، در نتیجه باعث از بین رفتن باکتری و منفی شدن کشت گردد. نتیجه&lrm; گیری: نتایج این پژوهش نشان دادند که شیوع آلودگی لپتوسپیرا در گاوهای استان مرکزی بسیار کمتر از دیگر مطالعات بود که به&lrm; همین دلیل امکان مقایسه دقیق سه روش باهم امکان &lrm;پذیر نبود، نکته جالب در این مطالعه، عدم همپوشانی تست &lrm;های مثبت در دو روش MAT و PCR بود که به &lrm;نظر می &lrm;رسد مطالعات بیشتری برای مقایسه دقیق‌تر این دو روش ضروری است. با توجه به نتایج این پژوهش، روش کشت ادرار به &lrm;دلیل طولانی بودن فرآیند آن و شرایط خاص کلونیزاسیون باکتری توصیه نمی&lrm; گردد.</div> <div style="text-align: justify;">Extended Abstract Background: Leptospirosis, caused by serovars of the bacterium Leptospira interrogans, is one of the most dangerous zoonotic diseases. It produces a wide range of sub-acute to acute clinical signs and can lead to infertility, abortion, reduced production, and significant economic losses to Iran's livestock industry. Leptospirosis diagnosis is of particular importance due to its potential risks to human communities, as the disease is easily transmitted by hosts such as rats and dogs, which are often found in significant numbers in livestock environments. However, since this disease can cause highly misleading symptoms in animals, such as mucosal jaundice, swollen lymph nodes, uveitis, hematuria, mastitis, a sharp drop in milk production and discoloration, abortion, birth of weak calves, weight loss, and fertility disorders, it increases the likelihood of diagnostic errors by veterinarians, thereby affecting the health status of human communities. Various laboratory methods can be used to diagnose the disease, including direct microscopic observation, culture, PCR, the Microscopic Agglutination Test (MAT), Enzyme-Linked Immunosorbent Assay (ELISA), and Immunofluorescence Assay (IFA). This research aimed to study and compare three methods—serology, culture, and PCR—for diagnosing bovine leptospirosis. Methods: In this study, blood and urine samples were collected from 400 heads of cattle with a history of at least one symptom of leptospirosis. Blood was drawn from the jugular vein under hygienic conditions. Serum samples were used for serological testing (MAT). Serum was separated from clotted blood tubes by placing them in a 37 °C water bath for 20 min, followed by centrifugation at 3000 × g for 10 min. To preserve the separated sera until the start of the MAT test, they were transferred using a sampler to sterile 2-mL microtubes and stored at -20 °C. The standard MAT procedure was as follows. The antigen used in this method (Leptospira interrogans) was cultured in a liquid medium for 7-10 days. The concentration of leptospires used was evaluated by counting in a chamber, and the volume was adjusted to contain 2 × 10^8 leptospires per milliliter. Urine samples were used for urine culture and PCR testing. Primers specific to L. interrogans were evaluated for the PCR assay. PCR reactions were set up in 25 µL volumes using a ready-made master mix (BIOFACT Company, Korea; composition: ddH2O: 5.50 µL, Buffer: 12.50 µL, Forward primer: 1.00 µL, Reverse primer: 1.00 µL, DNA: 5.00 µL). The required urine volume was 50 mL, collected in bottles containing 0.5 mL of filtered formaldehyde (0.45 µm). For bacterial culture and isolation, 5 µL of the collected urine was inoculated into a specific bacterial culture medium (semi-solid EMJH medium). Culture media were incubated at 30 °C for at least 5 weeks and examined every week for leptospiral growth using Dark-Field Microscopy (DFM). Data were analyzed using the Chi-square test and Fisher's exact test with SPSS statistical software version 23.0 (SPSS, Inc., Chicago, IL, USA, 2007). Significant differences between means were considered at a significance level of P < 0.05. Results: The MAT test was positive in five heads of cattle (1.2%). The most common serovars were Icterohaemorrhagiae (2 head = 40%), Grippotyphosa (2 head = 40%), and Canicola (1 head = 20%). All five samples that tested positive by MAT, plus all cattle showing clinical signs of leptospirosis, were candidates for PCR testing, four heads of which (1%) yielded a positive result. Urine cultures were prepared for all suspected cattle with positive MAT, PCR, and clinical signs. Despite the contamination of two culture media with environmental bacteria, none of the other media showed contamination, and no growth of Leptospira was observed (0%). Since animals in the early stages of the disease, whose immune systems have not yet completely cleared the bacteria from the blood, may not have allowed sufficient time for bacterial colonization in the kidneys; this could lead to negative urine samples. Additionally, Leptospira bacteria are sensitive to many antibiotics (treatment is doxycycline and azithromycin); therefore, these antibiotics may be used in cases where the animal has another disease, resulting in bacterial elimination and negative cultures. Conclusion: The results of this study indicated that the prevalence of Leptospira infection in cattle in Markazi Province was much lower than in other studies, which is why an accurate comparison of the three methods was not feasible. An interesting point in this study was the lack of overlap between positive tests in the MAT and PCR methods, suggesting that more studies are needed for a more precise comparison of these two methods.
